    Textbook Managers use this form to order required instructional materials based on new adoption and teacher or student growth.

    Complete all required areas. Secure textbook manager and principal signatures. Email, Maria Cruz or Fax (352) 742 – 6902.

     

     

    Discard

     

    Textbook Managers use this form to schedule pick up of off adoption textbooks, damaged textbooks, and no longer needed instructional materials from school campus.

     

    Complete all required areas. Email, Maria Cruz or Fax (352) 742 – 6902.

     

     

    Parents have an opportunity to review state adopted materials and have a voice to its use in the classroom.

    School Media Specialists review/discuss objection with principal and Challenge Review Committee members. Determine next steps in the process. Notify Faith Harris of school’s decision.

    End-of-Year Instructional Materials Checklist

     

    Textbook Managers are required to complete the end of year school inventory of instructional materials and submit this checklist before the last day of his or her contract.

    Complete all required areas. Attach lost textbook check with form. Secure textbook manager and principal signatures. Email/jackrabbit to: Faith Harris or Fax (352) 742 – 6902.

     New Course & Textbook Requisition Form

     

    Middle and high school administrators are required to complete the new course and textbook requisition form before opening new courses.

    Complete all requested areas. Include textbook’s title, ISBN, and quantity if needed. Secure school principal’s signature. Email/jackrabbit to: Faith Harris or Fax (352) 742 – 6902.
