Newborn Enrollment and/or Adoptions

You have 30 days from the date of birth to add your newborn to the Medical Insurance Plan.  You must enroll your newborn for at least the first 30 days for the birth, nursery and hospitalization charges to be covered.  There is no additional monthly premium for the first 30 days of coverage if enrollment is completed timely.

 If timely notice is not received, the applicable contribution will be charged from the date of birth. The applicable contribution for the child will be charged after the initial 30-day period in either case.  Coverage will not be denied for a newborn child if the Covered Plan Participant provides notice to School Board of Lake County Florida and the enrollment is completed within the 60-day period of the birth of the child and any applicable contribution is paid back to the date of birth.

If the newborn is not enrolled within sixty days of the date of birth, the newborn child will not be covered, and may only be enrolled under this Benefit Booklet during an Annual Open Enrollment Period, or in the case of a Special Enrollment event, during the Special Enrollment Period.

Enrollment Options

Medical Enrollment

If you plan to add your baby to the LCS health benefits, you will need to complete the enrollment process within thirty (30) days of the birth. You must call the Benefits Department as soon as you deliver to ensure you have ample time to complete the enrollment process.  Although the enrollment requires a social security number for dependents, you do not need to wait until the baby receives a social security number (SSN) to add him or her to your insurance. However, you should contact the Benefits Department at 352-253-6660 with the SSN upon receipt. 

TO ENROLL

You will log into the EMB Benefits Portal.  Username is lower case lcs plus your employee id number: Format Example lcs123456, Password: Date of Birth formatted YYYYMMDD. 

Flexible Spending Accounts

You may elect to participate in or change the amount you contribute to your Dependent Care Flexible Spending Account (DFSA). In order to do this, you will need to contact the Benefits Department and must complete changes to your account within the first 30 days after giving birth.

Life Insurance

You may consider purchasing life insurance coverage  for your child(ren). Be sure to update the beneficiaries indicated on your life insurance policy.

Taxes

 Review your federal and state W-4 withholdings to ensure you have the correct number of allowances and exemptions. Contact the payroll department for assistance.

    Make sure to update your beneficiary forms with FRS
