  • To register your child at Windy Hill Middle School....By Appointment Only
    1. Contact the school at (352) 394-2123 ext 4518 or 4519 and schedule an appointment.
    2. Complete enrollment forms (English Forms / Spanish forms);  or for school year 2019-2020, click here.
    3. Please bring the following items, if applicable, to the appointment.
    • Proof that you live in this school district/zone. (rental agreement, home purchase agreement, or utility bill)
      If living with someone else, please bring a notarized affidavit of residence, along with above items under the homeowner's name)
    • Name, Address, Phone and Fax Number of the Last School Attended
    • Final Report Card and/or withdrawal grades
    • FSA Scores/State Testing Scores/Standardized Test Results
    • LEP, 504, or IEP Plan (if applicable)
    • Child's Birth Certificate
    • Child's Social Security Card (optional)
    • Physical within the last year*
    • Immunization record* (See Immunization Requirements)
    *Please note students will not be able to start classes until proof of immunizations and a physical are submitted.
